Appointments/Immunization Schedule

Writing in an Appointment Book• Patients are seen by appointment. Nursing staff will provide families assistance and help in assessing if a sick-visit appointment is needed. Every attempt will be made to see a sick child on the same day they are ill, if warranted.

•Well-child visits should be scheduled well in advance of the time needed for the check-up. The usual schedule for routine check-ups and immunizations is as follows:

Exam Immunizations

Newborn- Hepatitis B#1, Newborn Screen#1 (in hospital).
2 weeks- Newborn Screen #2
1 month- Hepatitis B#2
2 months- Pentacel (DTaP#1, Hib#1, IPV#1), Prevnar#1, +Rota #1
4 months-Pentacel ( DTaP#2, Hib#2, IPV#2),Prevnar#2, +Rota #2
6 months-Pentacel ( DTaP#3, Hib#3, IPV #3),Prevnar#3, +Rota #3
9 months- Hepatitis B#3, CBC (anemia screen).
12months- (MMR#1/Varivax), Hepatitis A #1
15 months- DTaP/Hib#4, Prevnar#4
18 months- Hepatitis A#2
2 years- Usually no immunizations/Catch up on missed immunizations
3 years- Usually no immunizations/Catch up on missed immunizationsA Shot
4 years- DTaP#5, IPV#4, MMR#2, Varicella #2
5 - 10 years-Check up every 1-2 yrs. Usually no immunizations.
11 years Meningococcal, Tdap, +HPV (females only)
13-18 years- Check-up every 1-2 yrs. Usually no immunizations.

•Flu vaccinations may be given in the Fall to children ages 6 months to 18 years of age.

Preservative free vaccines are used at Kid Care Pediatric unless otherwise specified.

Texas Mandated Vaccine Changes for August 2009!

Hepatitis A Vaccine: Beginning August 1, 2009 all students entering kindergarten will be required to have two doses of Hepatitis A vaccine.

Measles, Mumps, and Rubella (MMR) Vaccine: Beginning August 1, 2009 all students entering kindergarten will be required to have two doses of Measles, Mumps, and Rubella (MMR) vaccine.

Varicella Vaccine: Beginning August 1, 2009 all students entering kindergarten and 7th grade will be required to have two doses of Varicella vaccine. A written statement from a parent, school nurse, or physician attesting to history of varicella disease or varicella immunity is acceptable in lieu of either dose of varicella vaccine)

Tetanus, Diptheria and acellular pertussis-containing vaccine (Tdap): Beginning August 1, 2009 all students entering the 7th grade will be required to have a booster dose of Tdap only if it has been 5 years since their last dose of tetanus-containing vaccine. More information here.

Meningococcal Vaccine: Beginning August 1, 2009 all students entering the 7th grade will be required to have one dose of Meningococcal vaccine.

Kid Care Pediatrics Policies

The following items comprise Kid Care Pediatrics policy:

A Sick KidCall the office at Keller (817) 337-5503 or Fort Worth (817) 847-6420 twenty-four hours prior to an appointment if the appointment is not going to be utilized/if the visit is being cancelled or postponed.

We reserve the right to charge $25 for appointment no shows without 24 hour cancellation notice.

Please be aware patients that arrive more than 5 minutes late for their appointments may be asked to reschedule.

Payments:

We collect all co-pays, deductibles, and co-insurance at the time of service according to the benefit quote that our office obtains from your insurance company.

We have a $30.00 fee for all returned checks.

After Hour Calls:

There is a $15.00 charge for all after hour urgent nurse triage calls.

For prescription refills, ask the pharmacy to forward a medication refill request to our office. For patients that are seen at the Keller office use (FAX # 817 337-0110) and for patients that are seen at our Fort Worth office use (FAX # 817-847-6412) or call the office for a refill request with 24-48 hours prior notice if possible in order to ensure a timely response. Holidays:

The office is closed for all major holidays. For urgent and emergency care, parents are to use the URGENT CARE and EMERGENCY CARE GUIDELINES previously listed.

Inclement Weather:

For inclement weather office hours, Kid Care Pediatrics follows the Keller/Fort Worth ISD school closure schedules. For urgent and emergency care when the office is closed due to inclement weather, please use the URGENT CARE and EMERGENCY CARE guidelines previously listed.

Kid Care Pediatrics physicians see patients from 0 – 18 years of age. Once 18 years of age, patients are referred to physicians that care for adults.

Kid Care Pediatrics providers believe in the efficacy of vaccines and strongly recommends patients receive state and school mandated immunizations in varying schedules. Elective vaccines are also offered. For families who will be refusing/declining vaccines for their children, Kid Care Pediatrics will kindly refer families to the Tarrant County Medical Society (PH# 817 732-2825) to search for other medical practices in agreement with an anti-vaccine medical philosophy.
